29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: Employees performing a roofing job at 6425 SW 6th Ave in Topeka, Kansas were exposed to fall hazards. The employees working on a roof with 12 in 12 slope were exposed to a fall distance of approximately thirteen (13) feet. In that a fall protection system was not being used while working on the roof.

1926.1053(b)(1) The side rails of a portable ladder shall extend at least 3 feet above the upper landing surface to allow access: Employees performing a roofing job at 6425 SW 6th Ave in Topeka, Kansas, were exposed to fall hazards of approximately 13.0 feet from the roof's eave. The portable ladder was not positioned in a manner to ensure the side rails extended at least 3 feet above the upper landing surface to allow employees safe access.
