29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.    On or about 17 January 2014, and at time prior thereto, the employer did not locate portable fire extinguishers so that they were readily accessible without subjecting the employees to injuries.   1) Next to the rollup door to the parking lot in the Plating Area, a fire extinguisher sign and bracket were present but the fire extinguisher was missing.  2) Next to the internal door from the Plating Area to Line 17, north side, a fire extinguisher sign and bracket were present but the fire extinguisher was missing.  3) Next to the internal door from the Plating Area to Line 17, south side, a fire extinguisher was mounted but the sign was missing.  4) In the Cleanroom alcove where the supplies are kept, a fire extinguisher sign and bracket were present but the fire extinguisher was sitting on the floor underneath.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel.    On or about 17 January 2014, and at times prior thereto, an employee was operating the Toyota 3000 Pound LPG Forklift #1 within the shipping and receiving area without the use of an operable horn, backup alarm, or backup lights, exposing the employees working or traversing in the area to hazards such as, but not limited to being struck by or crushed by the forklift.

On or about 17 January 2014, and at times prior thereto, employees performing work within Lines 13, 14 and 17 of the facility were utilizing a two-plug outlet in which a conduit connector to the outlet was loose with the internal wires visible and exposed to rubbing against the knockout opening, exposing the employees to an electrical hazard of approximately 120 volts.

On or about 17 January 2014, and at times prior thereto, employees performing work within Lines 13, 14 and 17 of the facility were utilizing a two-plug outlet in which the faceplate for the outlet was loose and hanging on a cord that was plugged in at the time with the internal wires and circuits of the outlet visible, exposing the employees to an electrical hazard of approximately 120 volts.

29 CFR 1910.305(j)(2)(iv): A receptacle installed in a wet or damp location was not suitable for the location.     On or about 17 January 2014, and at times prior thereto, a four-plug receptacle outlet within the Lab and adjacent to the sink was being used for wholesale testing by an employee that did not have ground fault circuit interrupter (GFCI), exposing the employee to an electrical hazard of approximately 120 volts.
