29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation.   At the inspection site, for calendar years 2011 through 2013, 2015, and 2016, incomplete information for at least 10 cases on the Log materially impaired understandability of the nature of hazards, injuries and illnesses in the workplace in that the type of injury was not recorded for 2 cases and the "object/substance that directly injured or made person ill" was not recorded for 10 cases. 1) For calendar years 2011-2013, 2015, and 2016, 10 cases did not have the "object/substance that directly injured or made person ill" recorded. 2) For calendar years 2015-2016, 2 cases did not have the type of injury and the "object/substance that directly injured or made person ill" recorded.

29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness.   At the inspection site, for calendar years 2011, 2012, 2015, and 2016, the OSHA 301 Incident Reports or substitute forms were incomplete and not maintained at the detail required by the regulation for each case on the OSHA 300 Logs. 1) For calendar year 2011, case#1 had a date of injury on line 11 as "3/23/2012" when it should have been "3/23/2011". Also, line 17 for the object or substance that directly harmed the employee was left blank, when it should have been filled out as "rack bar" or "bar". 2) For calendar year 2012, case# 1 did not have an OSHA 301 Incident Report. 3) For calendar year 2015, case# had a date of injury as "1" when it should have been "12/17/15". Also, line 17 for the object or substance that directly harmed the employee had "N/A", when it should have been filled out as "part". 4) For calendar year 2016, case# 1 had for the object or substance that directly harmed the employee on Line 17 as "N/A", when it should have been filled out as "panel".

29 CFR 1904.32(b)(2): The Summary of Work-Related Injuries and Illnesses (OSHA Form 300A or equivalent) was incomplete.  At the inspection site, for calendar year 2015, the OSHA Form 300A Summary of Work-Related Injuries and Illnesses was not completed or was incomplete. The Summary indicates there was a total of 1 injury on Line (1), but Lines (I) and (J) indicated a total of 2 recorded cases. Also, the form Year indicates 2003 but should be 2015.

At the inspection site, for calendar years 2011 through 2015, the OSHA Form 300A was not certified after the end of the year and prior to February 1st by a company executive demonstrating that the Log was examined and compared to the Summary to ensure correctness and accuracy.

At the inspection site, for calendar years 2011 through 2015, the OSHA Form 300A was not posted between February 1st and April 30th for the previous year. Calendar year 2013 had a certifying date of 4/23/2013, when it should be during 2014. Calendar year 2012 had a certifying date of 4/23/2013, which is beyond February 1st. And calendar year 2011 had a certifying date of 8/7/2012, which is beyond April 30th.

29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving information that a recordable injury or illness has occurred.   At the inspection site, for calendar years 2011, 2014 through 2016, the OSHA 301 Incident Report was not completed within 7 calendar days of the date of injury or illness for recordable incidents. 1) For calendar year 2011, cases# 1-6 exceeded the 7 calendar days, with all of them dated 8/7/2012 as the completion date for the OSHA 301 Incident Report. 2) For calendar year 2014, case# 1 did not have a completion date or a completed by recorded. 3) For calendar year 2015, case# 1 had a completion date of 1/2/2016 but an injury date as recorded on the OSHA 300 Log as 12/17/15. 4) For calendar year 2016, case# 1 had a completion date of 4/15/2016 but an injury date of 3/31/2016.
