29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  North, workstations #1, #2, #3, #4 electrical outlets, 2 Bright-Way R2620, 2 grey #4887, 2 unmarked 0336 black flexible cords: on January 21, 2014, employees were exposed to electrical shock hazards where listed and labeled electrical equipment was not installed and used in accordance with the manufacturers' provisions and other applicable listing standards for that type of equipment.   (a) Lower level pit's duplex outlet had 4 energized flexible extension cords, supplying power to workstations' outlets for heat guns, soldering stations and lighting use, plugged into its openings by a 3-way adapter risking possible circuit overload.   (b) 4 energized flexible extension cords running from lower level pit's duplex outlet were connecting to other 4 flexible cords supplying power to workstations' outlets where prohibited by manufacturers.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  (a) Southeast, workstation #4, CCI Seoprene 105 14 AWG 3/C SE00W E54864-H (UL) 600V flexible cable: on January 21, 2014, employees were exposed to electric shock hazards when stepping over energized flexible cable running on the floor from workstation #4 electrical box to the opposite south wall's outlet across the frequented walkway.  (b) North, workstations #1, #2, #3, #4, Bright-Way and other unmarked flexible cords: on January 21, 2014, employees were exposed to electric shock hazards when plugging-in/out and utilizing powered tools and other equipment connected to workstations' outlets supplied with power by energized flexible cords running from below the grade's pit outlet.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  Northeast, workstations #1 and #2, American Fluorescent Corporation 120V lighting fixture with Sylvania linear fluorescent tube light bulbs: on January 21, 2014, lighted bulbs in lighting fixtures were not guarded against accidental damage and contact where located below 8 ft. above the floor and worktables. Suitable enclosures, permanent and substantial partitions or screens were not used to protect employees from exposure to live parts.   Abatement certification is required of this item in accordance with the requirements of 1903.19(c).

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables may not be used where run through holes in walls, ceilings or floors:  North, workstations #1, #2, #3, #4, 2 Bright-Way R2620 and 2 grey #4887 unmarked flexible extension cords: on January 21, 2014, energized flexible cords were exposed to damages where running from workstations' outlets onto the floor and through the lower level pit's grates openings to the duplex outlet. Employees were plugging-in/out and utilizing powered tools and other equipment connected to workstations' outlets with power supplied by energized flexible cords running over the frequented grates walkway above the lower level pit's outlet.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
