29 CFR 1926.451(c)(1)(ii): Guys, ties, and braces were not installed according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and be repeated vertically at locations of horizontal members every 20 feet (6.1 m) or less thereafter for scaffolds 3 feet (0.91 m) wide or less: (a)JG Plastering LLC at 1906 28th St., Boulder, CO 80301: On or before January 15, 2014 the employer did not ensure the fabricated frame scaffold was tied into the building at a 4:1 height. The working surface of the scaffold was 13 feet 10 inches high and the scaffold was 3 feet wide. This condition exposed the employee to a fall of approximately 14 feet.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: (a)JG Plastering LLC at 1906 28th St., Boulder, CO 80301: On and before January 15, 2014, the employer did not ensure an employee working on a scaffold was protected from falls by the use of guardrails along all open sides and ends of the scaffold. The scaffold lacked end rails on the west end. This condition exposed the employee to a fall of approximate 14 feet.
