OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed-by and struck-by hazards while lifting loads with underhung cranes and overhead hoists.    a) Final Finish, 1004 Springbrook Avenue, Moosic, PA: Employees lifted loads in excess of three-hundred pounds (300#) with two (2) underhung cranes and four (4) hoists that had not been subject to frequent and periodic inspections within the last twelve (12) months. Employees were exposed to crushed-by and struck-by hazards, on or about January 22, 2014;    b) Shell Department, 1004 Springbrook Avenue, Moosic, PA: Employees lifted loads in excess of three-hundred pounds (300#) with four (4) underhung cranes and two (2) hoists that had not been subject to frequent and periodic inspections within the last twelve (12) months. The rated load on each of the two (2) hoists was marked as four-ton (4T) and the rated load on the crane bridge girder was marked as three-ton (3T). The combined load of both hoists exceeded the rated load of the crane bridge girder. Employees were exposed to crushed-by and struck-by hazards, on or about January 22, 2014;    c) Wall Set Department, 1004 Springbrook Avenue, Moosic, PA: Employees lifted loads in excess of three-hundred pounds (300#) with three (3) underhung cranes and three (3) overhead hoists that had not been subject to frequent and periodic inspections within the last twelve (12) months. An employee received injuries (broken neck bone, head laceration) from crane/hoist parts that fell more than twenty-foot (>20') while using a Budgit Hoist Model 5218E to set a wooden wall on February 19, 2014. Employees were exposed to crushed-by and struck-by hazards, on or about January 22, 2014;    d) Floor Department, 1004 Springbrook Avenue, Moosic, PA: Employees lifted loads in excess of three-hundred pounds (300#) with two (2) underhung cranes and two (2) hoists that had not been subject to frequent and periodic inspections within the last twelve (12) months. Employees were exposed to crushed-by and struck-by hazards, on or about January 22, 2014;; and    In accordance with 29 CFR 1903.19, abatement certification and documentation is required for this violation.

The spreader bar and lifting pins were not marked (or tagged) with the rated load or subjected to frequent and periodic inspections within the last twelve (12) months. The metal rod welded to the bottom of the spreader bar was showing evidence of wear. Employees were exposed to crushed-by and struck-by hazards, on or about January 22, 2014;    In accordance with 29 CFR 1903.19, abatement certification and documentation is required for this violation.

29 CFR 1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach:    a) Shell Department, 1004 Springbrook Avenue, Moosic, PA: Employees used alloy steel slings without permanently affixed durable identification stating size, grade and rated capacity, to lift a modular home unit roof. Employees were exposed to struck-by hazards due to the lack of identification that could be used to assure that loads beyond the slings rated capacity were not lifted, as observed on or about January 22, 2014.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards.    a) Plumbing & Sheathing Departments, 1004 Springbrook Avenue, Moosic, PA - Employees walked/worked from mezzanine and movable catwalk platforms with openings that were not guarded with standard railings. Employees were exposed to a fall hazard that varied between eight-foot and nine foot (8'-9') above ground level as observed on or about January 22, 2014.    b) Plumbing & Sheathing Departments, 1004 Springbrook Avenue, Moosic, PA - Employees worked from movable catwalk platforms with openings that were not guarded with standard railings due to missing posts, top rails and intermediate rails.  Employees were exposed to a fall hazard that varied between eight-foot and nine-foot (8'-9') above ground level as observed on or about January 22, 2014.    c) Plumbing & Sheathing Departments, 1004 Springbrook Avenue, Moosic, PA - Employees worked from movable catwalk platforms with openings that were not guarded with standard railings due to missing posts, top rails and intermediate rails.  Employees were exposed to a fall hazard that varied between eight-foot and nine-foot (8'-9') above ground level as observed on or about January 22, 2014.    d) Plumbing & Sheathing Departments, 1004 Springbrook Avenue, Moosic, PA - Employees worked on top of modular house units that had sides that were not guarded with standard railings. Employees were exposed to a fall hazards that varied between ten-foot and twelve-foot (10'-12') above ground level as observed on or about January 22, 2014.    The Signature Building Systems of Pennsylvania, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(c)(1), which was contained in OSHA inspection number 314162306, citation number 1, item number 1 and was affirmed as a final order on April 29, 2011, with respect to a workplace located at 1004 Springbrook Avenue, Moosic, PA.    In accordance with 29 CFR 1903.19(c) and (d), abatement certification and documentation is required for this violation.
