Employees were exposed to crushed-by and struck-by hazards, on or about January 22, 2014;    b) Shell Department, 1004 Springbrook Avenue, Moosic, PA: Employees lifted loads in excess of three-hundred pounds (300#) with four (4) underhung cranes and two (2) hoists that had not been subject to frequent and periodic inspections within the last twelve (12) months. The rated load on each of the two (2) hoists was marked as four-ton (4T) and the rated load on the crane bridge girder was marked as three-ton (3T). The combined load of both hoists exceeded the rated load of the crane bridge girder. Employees were exposed to crushed-by and struck-by hazards, on or about January 22, 2014;    c) Wall Set Department, 1004 Springbrook Avenue, Moosic, PA: Employees lifted loads in excess of three-hundred pounds (300#) with three (3) underhung cranes and three (3) overhead hoists that had not been subject to frequent and periodic inspections within the last twelve (12) months. An employee received injuries (broken neck bone, head laceration) from crane/hoist parts that fell more than twenty-foot (>20') while using a Budgit Hoist Model 5218E to set a wooden wall on February 19, 2014. Employees were exposed to crushed-by and struck-by hazards, on or about January 22, 2014;    d) Floor Department, 1004 Springbrook Avenue, Moosic, PA: Employees lifted loads in excess of three-hundred pounds (300#) with two (2) underhung cranes and two (2) hoists that had not been subject to frequent and periodic inspections within the last twelve (12) months. Employees were exposed to crushed-by and struck-by hazards, on or about January 22, 2014;; and    In accordance with 29 CFR 1903.19, abatement certification and documentation is required for this violation.
