29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    On or about January 21, 2014, and times prior to, at 100 Four Paws Lane in Maumelle, Arkansas,    (a)  there was an excessive accumulation of miscellaneous items such as but not limited to boxes, a cooler and hoses scattered throughout the floor in the upstairs storage room.    (b)  there was an excessive amount of cookie dust on the floor in the Kitchen area.    (c)  the flexible cord to the Rotary Cookie Machine located in the Kitchen area was lying on the floor.    These conditions exposed employees to slips, trips and falls.

29 CFR 1910.147(a)(2)(iii)(A): Work on cord and plug connected electric equipment for which exposure to the hazards of unexpected energization or start up of the equipment is controlled by the unplugging of the equipment from the energy source and by the plug being under the exclusive control of the employee performing the servicing or maintenance:    (a)  On or about January 21, 2014, at 100 Four Paws Lane in Maumelle, Arkansas, in the Kitchen area, the employer did not ensure employees unplugged and maintained control of the cord plug to the ProBake Rotary Cookie Machine prior to performing equipment cleaning tasks.  This condition exposed employees to caught-in moving parts if the machine was unexpectedly energized.    (b)  On or about January 8, 2014, at 100 Four Paws Lane in Maumelle, Arkansas, in the Kitchen area, the employer did not ensure employee unplugged and maintained control of the cord plug to the Rondo Sheeter Machine prior to performing servicing tasks such as but not limited to adjusting the belt over the pulley.  This condition exposed employee to being caught between the belt and pulley if the machine was unexpectedly energized.
