29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: (a) Allegheny Ludlum Hot Rolling Processing Facility, 100 North River Road, Brackenridge, PA 15014: On or about January 17, 2014, three employees were removing a defective filter/transformer/blower module to the C-8182 overhead crane and were replacing it with a new one. Only one of the three authorized employees had his lock in place to lock out the overhead crane.
