29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: a) Worksite, West Mezzanine: Employee assigned to perform repair on Formost Packaging Machine #008 (Model #JV-6086WRH / Serial # 03JV33W) was exposed to the unguarded sprocket wheel and chain; on or about 1/28/14. NOTE: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
