Personnel utilized a forklift and pallet as a man lift when adding acid to the tanks in the plating area. Personnel would be lifted to a height of approximately 4 to 5 feet in the air by the forklift while completing task.

The unguarded nip points included but were not limited to the nip points created by the sanding belt and rotating wheels that drive the sanding belt.         b) Advanced Mold Technologies, Inc. dba Advanced Custom Chrome / 2011 East 30th Street, Erie, PA: On or about January 29, 2014, the Mitchell Engineering Company belt sander did not have guarding to protect employees from moving/rotating pats. The unguarded rotating/moving parts included but were not limited to the: (1) Rotating spindle nut located on the outside of the lower sand belt wheel.  (2) The top rotating drive wheel for the sanding belt.

The cord was routed under a work mat on a floor and cord also routed between a board and walkway.  The condition potentially exposed to the cord to abrasions and pinchpoints.  The cord had been in use to operate a wet/dry shop vacuum in the area.
