29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:     (a) Advanced Mold Technologies, Inc. dba Advanced Custom Chrome, 2011 East 30th Street, Erie, PA: On or about January 29, 2014 employees used disposable 3M brand tight fitting, half mask respirators with combination cartridges on a voluntary basis during electroplating operations.  The company had not developed a written respiratory protection program to describe medical evaluation requirements/procedures, and cleaning and storage guidelines.

The self contained emergency eye wash station was not maintained according to the manufacturer's requirements, in that the unit was not drained and refilled with water, and the required preservative solution, every three months.

29 CFR 1910.1026(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI):    (a) Advanced Mold Technologies, Inc. dba Advanced Custom Chrome, 2011 East 30th Street, Erie, PA: On or about January 29, 2014, and at times prior thereto, employees conducting plating operations utilizing Catachrome DEC plating solution, which contained hexavalent chromium.  The company had not conducted air monitoring to determine employee exposure levels to hexavalent chromium.
