29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a. On or about 01/29/2014, at the job site - employees were permitted to work from a scaffold that was not fully equipped with all the necessary guardrails, exposing the employees to a 26 foot and 19 foot 6 inches fall hazard. Mid-rails were missing along the front, top and mid-rails were missing at the ends.
