29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  a. On or about February 15, 2017, at the west side of the residential building, three employees using pneumatic nail guns to install wood on the fascia of a 6:12 pitch roof were not wearing eye protection, and exposed to flying debris and struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  On or about February 15, 2017, at the north side of residential building No.8, three employees doing sheathing work on a 6:12 pitch residential roof were not protected from a 30ft-4in. and 15ft. fall hazards by the use of a fall protection system.     b.  On or about February 15, 2017, at the west side of residential building No.8, three employees doing wood bracing work for the fascia while standing on the top plate at the leading edge were not protected from a 20 feet fall hazard by the use of a fall protection system.
