29 CFR 1910.67(c)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. On the parking lot, East side, on or about January 31, 2014, an employee without fall protection was exposed to a fall hazard greater than 6 feet, while using an Nifty aerial lift to replace ballasts and light bulbs on the parking light fixtures.

29 CFR 1926.454(a): The employer did not have each employee that was performing work on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: On the parking lot, East side, on or about January 31, 2014, an employee without fall protection was exposed to a fall hazard greater than 6 feet, while using an aerial lift without proper training on how to operate, and recognize potential hazards while installing and replacing ballasts and light bulbs on the parking light fixtures.
