29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation:  On or about July 24, 2023, and at times prior thereto, in Building A (Warehouse Department), an orange (16 AWG) extension cord used to supply power to a Porta cool Jetstream 36# Variable Speed PACJ2601A1 120VAC/60HZ 9.3 APS Serial #531849, was damaged and had insulation cuts with exposed inner conductors. This condition exposed employees to electrical shock hazards.

Soldered splices were not first spliced or joined to be mechanically and electrically secure without solder and then soldered. All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose:  On or about July 24, 2023, and at times prior thereto, in Building A (Warehouse department), the splices on a Porta cool Jetstream 36# Variable Speed PACJ2601A1 120VAC/60HZ 9.3 APS Serial #531849 were not spliced or joined together by brazing, welding, or soldering with a fusible metal or alloy. This condition exposed employees to electrical shock and fire hazards.

The extension cord was being used to power a Porta cool Jetstream 36# Variable Speed PACJ2601A1 120VAC/60HZ 9.3 APS Serial #531849. This condition exposed employees to electrical shock hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  On or about July 24, 2023, employees were exposed to electrical hazards when damaged / defective extension cords were used to power equipment in the following instances:  1) Building A (Warehouse) a (16 AWG) orange extension cord was run through the top of a cabinet door and was being pinched and had insulation cuts.   2) Building A (Warehouse department), the splices on the Porta cool Jetstream 36# Variable Speed PACJ2601A1 120VAC/60HZ 9.3 APS Serial #531849, were not spliced or joined together by brazing, welding, or soldering with a fusible metal or alloy.
