29 CFR 1910.147(f)(1): When Lockout or Tagout devices were temporarily removed from the energy isolating device(s) and the machine or equipment was energized to position and / or test the machine, equipment or components thereof after repair(s) / servicing; the sequence of actions were not followed: 1910.147(f)(1)(i)- clear the machine or equipment of tools and materials in accordance with paragraph (e)(1) of this section West Point Dairy Products, LLC: The employer failed to ensure that the work area was inspected to ensure that nonessential items had been removed prior to removing Lockout / Tagout devices and restoring energy to Hyster Model # S50XL, Serial# C187V10894R after a water pump was installed. The mechanic was testing the installation and reached with his left hand for his flashlight that was on the cowling on the rear part of the forklift and made incidental contact with the moving fan blade.
