29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, was not guarded by a floor opening cover of standard strength and construction.  While the cover is not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings:  a.  On or about January 30, 2014,  in the maintenance area,  an approximately 80 foot long pit was not guarded by a standard guard railing on all exposed sides.  Employees were exposed to an approximately 5 foot 6 inch fall hazard while performing maintenance on a locomotive directly above the pit.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  a.  On or about April 18, 2014,  in the running side of the maintenance shop, employees performing maintenance activities on locomotives such as but not limited to sanding and inspection are exposed to an approximately 8 foot fall hazard while working on fold down platforms missing standard railings.
