29 CFR 1917.42(i)(2): Screw pin shackles, used aloft in house fall or other gear, did not have their pins moused or otherwise effectively secured. (a) Starvin Marvin pedestal crane: The Unic Pedestal crane S/N V021141 used to off load vessels had a screw pin shackle that was not positively secured. This exposed employees to struck-by hazards if the pin backed out during unloading operations.
