29 CFR 1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of job-sites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):       (a)    Aztec LLC dba Aztec Builders, at 1308 W. Plum St., Fort Collins, CO 80521:  On or before January 21, 2014, the controlling employer did not ensure that employees were protected from injury, in that an employee was engaged in roofing activities on a steep roof (4.5 in 12 pitch) of an apartment building under construction without the use of fall protection.  This hazard exposed the employee to a fall of approximately 43 feet and the employee did fall this distance and sustained serious injuries.
