29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers (i.e. a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them): (a)Aztec, LLC @ 970 28th Street, Boulder, CO 80303: On and before February 20th, 2015, the employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person (i.e. a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them). An employee of Aztec was rolling out floor joists on a multi-family structure under construction while wearing a full body harness and not attached to a fall protection system. Aztec did not maintain a safety program which included frequent and regular inspections of the jobsite. This condition exposed an employee to a fall hazard to the interior of at least 10 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Aztec, LLC @ 970 28th Street, Boulder, CO 80303: On and before February 20th, 2015, the employer did not ensure that each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. An employee was in the process of rolling floor joist without utilizing a fall protection system. This condition exposed employees to fall hazards of at least 10 feet.
