29 CFR 1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged: (a) Facility, 9558 Route 22 W, Blairsville, PA: On or about 02/03/14 the load on a Kalmar AC BV158, Model P90CX, Serial #102749A was not stable and safely arranged. The operator was moving a rebar cage approximately 9'5" W x 11'5" L x 8' H that was secured to an attachment with a four leg sling. The truck had the boom fully extended. During travel the operator became stuck in a sloped drain inlet and the truck was on a slant. The operator attempted to move the truck forward and back but could not get free. The load was not lowered to the ground and began to swing. The swinging caused the center of gravity to shift causing the forklift to tip over. The employee sustained broken vertebrae in his neck from the impact.
