29 CFR 1910.184(e)(1):Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach: a) Facility located at 31 East Bridge Street Spring City, PA 19475 - On or about April 27, 2023, the employer did not ensure 3/8" and 1/2" metal alloy chain slings had permanently affixed durable identification stating size, grade, rated capacity, and reach. Employees were directed to use chain rigging slings with missing identification tags to move materials throughout the facility, thus exposing employees to struck-by hazards.

29 CFR 1910.184(e)(9)(i):Alloy steel chain sling(s) with cracked or deformed master link(s), coupling link(s), or other component(s) were not removed from service: a) Facility located at 31 East Bridge Street Spring City, PA 19475 - On or about April 27, 2023, the employer did not ensure alloy steel chain slings with defective components, including but not limited to seized coupling links and broken latches, were removed from service. Employees were directed to use chain rigging slings that had been tagged out of service to move materials throughout the facility, thereby exposing employees to struck-by hazards.
