29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    (a)  On or about January 29, 2014, the employer did not ensure employees were protected from shock hazard from a green extension cord that was in use to power a hot air welder, that had it insulation pulled back from the plug exposing the wires inside.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  (a)  On or about January 29, 2014, the employer did not ensure employees were protected from shock hazards from an extension cord whose insulation was cut exposing the conductors inside and was laying in the snow.  It was connected to a hot air welder that was in use on the roof of the building under construction.  Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    (a)  On or about January 29, 2014, the employer did not ensure that two employees were protected from falls of greater than 20 feet while they were stripping-in the cover cap on the exterior edges of the roof and parapets of the building under construction.    Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").    MK Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 29552, citation number 1, item number 1, and was affirmed as a final order on July 6, 2011, with respect to a workplace located at 4550 East Jewell Avenue in Denver, Colorado.
