29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a. On or about Febraury 6, 2014, the Werner extension ladder used to access and egress the top of a structure, did not extend 3-foot above the eave, exposing an employee to a 10-foot fall hazard.

Conin Framing Group, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 951877, citation 02, item number 001, issued on January 30, 2014 with respect to a workplace at 12565 Dewhurst Circle, Jacksonville, Florida 32246.The final order date was February 21, 2014.    29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a. On or about February 6, 2014, on top of a structure, an employee using a Hitachi pneumatic air nail gun was not wearing safety glasses, exposing himself to being hit by nails or flying debris.

The final order date was June 19, 2013.    29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     a. On or about February 6, 2014, on top of a structure, an employee performing framing operations was not protected from a 10-foot 7-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.
