29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: (a) Facility, Press Room, 425 Locust Street, Johnstown, PA: On or about 02/05/14 each authorized employee performing maintenance on the Goss Metro Offset press, unit 5 did not affix a personal lockout or tagout device before working on the equipment. Three persons were performing maintenance and only one lockout device had been applied to control the energy source.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Facility, Mail Room, 425 Locust Street, Johnstown, PA: On or about 02/05/14 machine guarding was not provided on the H unit of the Harris 848 inserter exposing the operator at that station to the rotating parts. (b) Facility, Mail Room, 425 Locust Street, Johnstown, PA: On or about 02/05/14 adequate machine guarding was not provided on the D and G units of the Harris 848 inserter in that the plexiglass guards were broken and did not fully protect the operator from the rotating parts.
