29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  All production departments, plastic injection molding presses and secondary production machines: on February 6, 2014, employees locking out machinery for servicing and other setup tasks were not inspected periodically (at least once per year) to ensure their level of proficiency in the hazardous energy control procedures implemented by the employer to ensure sufficient level of protection and compliance.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  East, toolroom department, Bridgeport VMC 1000 XP serial no. 822504 vertical milling machine, Haas VF-0E serial no. 16672 vertical milling machine: on February 6, 2014, an employee operating and performing production-related setup tasks was exposed to cuts, lacerations, caught-in, and struck-by hazards where the safety interlocks were by-passed and otherwise not operational as designed and constructed. Alternative methods of protection to prevent employees' access to the point of operation and associated danger zone were not provided.  (a) Bridgeport VMC 1000 XP serial no. 822504 vertical milling machine had a by-passed key-type safety interlock during setup and operation, allowing machine to run its automated operations with operator's access doors open.  There is no abatement certification or documentation required for this item.  (b) Haas VF-0E serial no. 16672 vertical milling machine had two by-passed safety interlocks and operated with a door hold safety feature programmatically overridden to allow opening of the operator's access doors during operation and setup without assuring other effective protection.  Abatement certification is required of this item in accordance with the requirements of 1903.19(c).
