29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Molding Room, Cap Stacking and Packaging Machine (TSD 21410)- On or about August 18, 2014, the point of operation in the stacker was not adequately guarded to prevent employees from reaching into the danger zone to reposition boxes during operating cycles. Employees were exposed to struck-by and/or caught in-between hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
