29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    (a) Building 1-2:  Employees were exposed to tripping hazards from debris around the building and cords tangled in the hallways inside of the building, as determined on February 4, 2014.      (b) Building 2-2:   Employees were exposed to tripping hazards from debris, trash, other material, and hoses from compressor in walkways, as determined on February 4, 2014.      (c) Building 111:  Employees were exposed to tripping hazards from a timber on the ground in front of the entrance, as determined on February 4, 2014.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.      (a) Building 2-2:  Employees were exposed to falls of 8 feet to the floor of the second story while walking interior walls installing the top plate to a residential student dormitory, as determined on February 4, 2014.

(a) Morgantown, WV:  Employees did not receive training that enabled them to recognize the hazards of working from heights, as evident by the employees being observed walking the interior walls (2X4) installing the top plate without the use of a fall protection system exposing the employee to falls of 8 feet, as determine February 4, 2014.
