29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    Jobsite: Warehouse - charger marked circuit #24.    An employee was shocked and injured when the power supply "cord", which had been fashioned from flexible metallic conduit, single conductors, and a nylon twist-lock plug, and was energized with a 480 volt circuit, had been crushed, which later caused a short within.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Jobsite: Warehouse - chargers marked #'s 24 & 26.  An employee was shocked and seriously injured while working with and around electrical equipment that was not installed and used as the manufacturer and testing laboratory instructed:  a) A flexible metallic conduit had been crushed, damaging the conductors within and had not been taken out of service and/or repaired.  This damage later shorted the 480 volt circuit, causing this incident. b) Twist-lock plugs, meant for use with rubber cord, had been attached to flexible metallic conduits. c) Improper connectors had been used to attach flexible metallic conduit to its point of termination at the chargers' junction box. d) Anti-short bushings had not been installed on the ends of the flexible metallic conduits. e) Support straps had not been installed within 1 foot of the terminations to prevent strain on the conduit-to-fitting and the fitting-to-termination connections. f) The concentric knockouts of the chargers, which had not been listed by an authorized laboratory to serve as a grounding means, were used as the sole means as safety ground for the flexible metallic conduit.  All of the above are required by the NFPA 70, NEMA Installation Guide, and UL Listings of the products.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:    Jobsite: Warehouse.    Employees were exposed to electrocution and other hazards where the electrical panel, powering eleven (11) fork truck battery chargers, among other equipment, was not marked to indicate the purpose of the individual overcurrent devices.
