29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a. East side of building - an employee was exposed to a 14'-1" fall hazard, in that, the mid-rails were missing along the east side of the scaffold. The top and mid-rails were missing at the ends of the scaffold. Violation observed on or about 2/11/14.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a. At the site - employees exposed to fall hazards were not provided with training to recognize hazards associated with working on a scaffold. Employees did not know at what height fall protection was required while on a scaffold. Employees did not know what was needed for a guardrail system and did not know that a cross brace could only serve as a top or mid-rail, but not both. Employees did not know at what height the top and mid-rails needed to be installed. Violation observed on or about 2/11/14.
