OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from struck by hazards caused by the movement of the bridge girder form work when vertical lifting components were removed prior to securing or bracing the bridge girder formwork.    a) At the bridge girder line; Vertical lifting devices were removed from a bridge girder form prior to adequately bracing or securing the formwork to prevent movement. On or before January 21, 2014, an employee suffered a crushing injury that resulted in a partial leg amputation as a result of the form tipping and pinning the employee between the form and a concrete wall.
