29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard. a) On or about 6/8/2016, ZUBLIN Wire Machine was not guarded below cage being formed which left an unguarded opening to a pit below approximately 5' deep. b) On or about 6/8/2016, a guard railing was needed on the platform for the stairs at the 2nd to third flight where the crane rail is located with approximately a potential 20' to 25' fall to the concrete floor. There was a gap of approximately 7' from the existing hand rail to the I beam.
