29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      In the coating head enclosure of coater #2:  On or about February 6, 2014, an employee cleaning a rubber roller in the coating head enclosure portion of coater #2 caught his left arm in an in-going nip point between a rubber roller and a feed roller which was not protected by one or more methods of machine guarding.   Adhesive Applications, Inc. was previously cited for a violation of the same standard which was contained in OSHA inspection 315515684, citation number 1, item number 1, and was affirmed as a final order on February 1, 2012, with respect to a workplace at 218 Race Street, Holyoke, MA.

29 CFR 1910.106(e)(2)(iv)(d): Flammable liquids were not drawn from or transferred into vessels, containers, or portable tanks within a building only through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tanks by gravity through an approved self-closing valve:      In the flammable liquid storage and adhesive mixing room:  On or about May 1, 2014, an employee filled a metal, open-top 5 gallon container with toluene, a category 2 flammable liquid, and then transferred the flammable liquid from the metal, open top 5 gallon container into a 55 gallon drum.  The 5 gallon container was not a safety can and did not have an approved self-closing device nor was the process performed through a closed piping system.

29 CFR 1910.106(e)(6)(ii): A category 2 flammable liquid was dispensed into containers without the nozzle and container being electrically interconnected:  (a)  In the flammable liquid storage and solvent/adhesive mixing room:  On or about May 1, 2014, an employee dispensed a category 2 flammable liquid, toluene, from a metal, 55 gallon drum, which was grounded, into a metal, open-top 5 gallon container without bonding the 5 gallon container to the 55 gallon drum.     (b)  In the flammable liquid storage and solvent/adhesive mixing room:  On or about May 1, 2014, an employee dispensed a category 2 flammable liquid, toluene, from a metal, 5 gallon open-top container to a grounded, metal 55 gallon drum of a flammable adhesive mix without bonding the 5 gallon container to the 55 gallon drum.
