29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Adhesive Applications, 51 Ferry St., East Hampton, Massachusetts; On and before March 15, 2019 the employer did not have a written energy control program that included written energy control procedures and periodic inspections for the various machines in the facility including but not limited to baloney slitters. Employers are exposed to laceration and amputation hazards when performing servicing, maintenance, or set up on a machine or piece of equipment, since the machine or equipment is not isolated from the energy source and rendered inoperative.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Adhesive Applications, 51 Ferry St., Easthampton, Massachusetts: On and prior to March 15, 2019 employees were exposed to laceration and amputation hazards: a. employees operating the E420NM5 baloney slicers were exposed to lacerations and amputation hazards as the cover guards had holes cut into them to allow access to the product without having to shut the machine off or lock it out. b. Lever baloney slicer in the back by the windows was not guarded to prevent access to the blades either from the front where the operator stands or in the rear of the machine while it was in operation.
