29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity: a) On or about 2/13/14 on lower level of former church being renovated into apartments at 400 Lincoln Pkwy., Buffalo, NY: Employee used a Hilti Model TE706 chipper to break up concrete. The chipper was connected to 120 volt-ac power via a series of extension cords. The last extension cord in the series that the chipper was actually plugged into, had reversed polarity. NO ABATEMENT CERTIFICATION REQUIRED

The chipper was connected to 120 volt-ac power via a series of extension cords. The last extension cord in the series that the chipper was actually plugged into was cut/frayed exposing the conductors. NO ABATEMENT CERTIFICATION REQUIRED
