29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: A.) Pennsylvania Sintered Metals, Inc. dba PSM Brown Company - On or about 2/13/14, the company did not have a lockout/tagout program consisting of specific lockout/tagout procedures, authorized employee training, and periodic inspections. Employees perform maintenance, servicing and repairs on machinery including but not limited to, molding presses and sintering furnaces.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A.) Pennsylvania Sintered Metals Inc. dba PSM Brown Company - On or about 2/13/14, the interlocks on the CNC lathes were not operational. Employees were able to open the door at any time in the cycle and the machine would not stop running. The CNCs with the malfunctioning interlocks included but were not limited to D-1, M-1, M-2, M-6, M-7, M13, and M14.
