29 CFR 1910.23(a)(3): Every hatchway and chute floor opening was not guarded:       a) During the period of approximately September 18, 2013 and February 18, 2014, at the above located workplace, employees were required work on one or both of the work platforms located just west and above silo 23. Both work platforms had unguarded floor openings that measured approximately 2.5 feet by 6 feet.  The employees were thereby exposed to a fall hazard of over 20 feet.          Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

The floor hole measured approximately 6 inches by 6 inches. Employees were thereby exposed to trips and falls hazards.            b) On or about February 18, 2014, at the above located workplace, employees were required to work around a broken floor drain cover/hole on the work platform at the midlevel and just west of silo 23. The floor hole measuring approximately 3.5 by 5 inches. Employees were thereby exposed to trips and falls hazards.
