29 CFR 1910.23(a)(3): Every hatchway and chute floor opening was not guarded: a) During the period of approximately September 18, 2013 and February 18, 2014, at the above located workplace, employees were required work on one or both of the work platforms located just west and above silo 23. Both work platforms had unguarded floor openings that measured approximately 2.5 feet by 6 feet. The employees were thereby exposed to a fall hazard of over 20 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded by either a standard railing or floor hole cover: a) On or about February 18, 2014, at the above located workplace, employees were required to work around a floor hole on the work platform at the upper level and just west of silo 23. The floor hole measured approximately 6 inches by 6 inches. Employees were thereby exposed to trips and falls hazards. b) On or about February 18, 2014, at the above located workplace, employees were required to work around a broken floor drain cover/hole on the work platform at the midlevel and just west of silo 23. The floor hole measuring approximately 3.5 by 5 inches. Employees were thereby exposed to trips and falls hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) During the period of September 18, 2013 to February 18, 2014, employees at the above located workplace were required to work with and around a 120-220 volt screw conveyor. The screw conveyor had a "LB" (90 degree wire pulling elbow) which did not have a cover. The condition of the elbow (uncovered with exposed wires) was not in accordance with instructions included in the listing or labeling. Employees that worked at or around the floor screw conveyor were thereby exposed to an electrical hazard.

The screw conveyor had a "LB" (90 degree wire pulling elbow) which did not have a cover. The wires in the elbow were exposed and energized. Employees that worked at or around the floor screw conveyor were thereby exposed to an electrical hazard.
