29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about 2/10/2014, at the above addressed jobsite, an employee was exposed to flying particles while using a circular saw without wearing eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about 2/10/2014, at the above addressed jobsite, three employees were exposed to fall hazards while conducting carpentry work without having a fall protection system in place.  Instance 1:  Two employees installing 4 x 8 plywood onto the roof trusses. Instance 2:  One employee installing 2x4's on a small roof canopy.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about 2/10/2014, at the above addressed jobsite, three employees were exposed to fall hazards while using an extension ladder where the side rails did not extend at least 3' from the 2nd floor landing.

29 CFR 1926.1053(b)(17): Fixed ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, or corroded components, were not withdrawn from service until repaired:  On or about 2/10/2014, at the above addressed jobsite, three employees were exposed to fall hazards while using an extension ladder that was not withdrawn from service due to its missing safety shoes.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:  On or about 2/10/2014, at the above addressed jobsite, a Werner extension ladder was not inspected by a competent person for visible defects on a periodic basis whereas the ladder had missing safety shoes and was not extended 3' or more above the 2nd floor landing.   PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
