29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s): (a) Boiler Room, US Border Patrol Tucson Station, 2430 S Swan Rd., Tucson, AZ 85711: On or about 12 February 2014, and at times prior thereto, two knife switch disconnects (not marked as to location and purpose), installed on the far north end of the boiler room, were locked and tagged out, without the devices having any identification of who applied the lockout devices or when, exposing the employees to hazards.
