29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  (a) US Border Patrol Tucson Sector, 2430 S Swan Rd., Tucson, AZ 85711:  On or about 12 February 2014, and at times prior thereto, the employer did not verify that a workplace hazard assessment had been performed, which necessitated use of personal protective equipment (PPE), exposing the employees to hazards.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) Boiler Room, US Border Patrol Tucson Station, 2430 S Swan Rd., Tucson, AZ 85711:  On or about 12 February 2014, and at times prior thereto, the employer did not provide suitable quick drenching or flushing faciltiies for emergency use where a corrosive liquid (potassium hydroxide liquid in a half empty 10 gallon barrel) was potentially used, exposing employees to hazards.

29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s):  (a) Boiler Room, US Border Patrol Tucson Station, 2430 S Swan Rd., Tucson, AZ 85711: On or about 12 February 2014, and at times prior thereto, two knife switch disconnects (not marked as to location and purpose), installed on the far north end of the boiler room, were locked and tagged out, without the devices having any identification of who applied the lockout devices or when, exposing the employees to hazards.
