OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  (a) F/V Beauty Bay, Engine Room:  On or about 11 February 2014 and times prior thereto, Distribution- Panel B missing a blank or protective cover.  This condition exposed employees to a 120/208 volt electrical shock and burn hazard.    (b) ) F/V Beauty Bay, Foc'sle:  On or about 11 February 2014 and times prior thereto, Factory Distribution Panel missing a blank or protective cover.  This condition exposed employees to a 120/208 volt electrical shock and burn hazard.  Among other methods, one feasible and acceptable method to correct this hazard includes but not limited to install approved protective cover as per the manufactures recommendation.

29 CFR 1915.134(b): Work rests were not kept a distance not to exceed 1/8 inch from the surface of the wheel.  F/V Beauty Bay, Engine room:  On or about 11 February 2014 and times prior thereto, the 6 inch Craftsman bench grinder work rest measured approximately 5/8 inch from abrasive wheel.  This condition exposed employees to a struck by and caught in hazards.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.  F/V Beauty Bay, Engine room:  On or about 11 February 2014 and times prior thereto, the 6 inch Craftsman bench grinder was missing the tongue guard.  This condition exposed employees to a struck by hazard.

29 CFR 1915.152(b)(4): The employer failed to document an occupational hazard assessment which should have included: Occupations involved in the survey, the date in which the survey took place and the name of the person who performed the hazard assessment.    F/V Beauty Bay:  On or about 11 February 2014 and times prior thereto, the employer did not have a written hazard assessment for employees to clean the factory area using Orcachlor Chlorine and Harbor Master All-Purpose Cleaner Concentrate.  This condition exposed employees to skin irritation, eye irritation and respiratory distress.

29 CFR 1918.11(a)(1): The employer did not have annual examinations of gear certification under ILO 152 after July 27, 1998.      F/V Beauty Bay, Coastal dock:  On or about 11 February 2014 and times prior thereto, Ship deck pedestal crane was used for off-loading operations, without ascertaining that the crane had a current annual inspection. Last inspection was conducted by Hansen Boat Company in August 2012.  This condition exposed employees to a struck-by hazard.

F/V Beauty Bay:  On or about 11 February 2014and times prior; the company executive did not ensure that the OSHA 300A Summary of Work-related Injuries and Illness Log was accurate and complete, and/or deficiencies were identified and corrected at the end of the calendar year. Two instances were noted:             Instance (a) 2013 - No OSHA Form 300A or its equivalent was not completed.       Instance (b) 2012 - No OSHA Form 300A or its equivalent was not completed.        Instance (b) 2011 - OSHA Form 300A  did not have the total number of employees and hours worked for the year.
