29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: Factory and Engineering Spaces: On or about February 11, 2014 and at times prior thereto, where chemicals such as Harbormaster and Orcachlor were used, the employer had not developed, implemented, and maintain a written hazard communication program. This condition limited information on chemicals in the workplace and exposed employees to eye and skin burn hazards from contact with the corrosive cleaners and sanitizers.

29 CFR 1915.89(c)(1): The employer did not ensure the use of a lock to prevent energization or startup, or the release of hazardous energy, before any servicing was started. Factory: On or about February 11, 2014 and at times prior thereto, where equipment was serviced including the blade removal of the Tokai Head Cutter PHC-550L, the employer had not ensured that the equipment had been deenergizied. This condition exposed employee to finger amputation hazrds in the event of inadvertent start-up.
