29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:       A. Wells Construction worksite located on Thistledown Lane (Lot 149), Perrysburg, Ohio: On or about February 13, 2014, the employer did not ensure an employee was wearing proper eye protection while operating a pneumatic nail gun while performing framing activities. The employee was exposed to potential flying object hazards while operating the pneumatic fastener.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.        A. Wells Construction worksite located on Thistledown Lane (Lot 149), Perrysburg, Ohio: On or about February 13, 2014, the employer did not ensure employees were protected from head injuries while working below employees installing wood rafters. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    A. Wells Construction worksite located on Thistledown Lane (Lot 149), Perrysburg, Ohio: On or about February 13, 2014,  the employer did not ensure employees were protected from falls while installing roof sheeting on the south side of the two story residential structure. Employees were exposed to a fall distance of approximately 19.00 feet to the lowest level from the 7/12 pitch roof.
