29 CFR 1926.405(b)(2): Covers of outlet boxes having holes through which flexible cord pendants pass were not provided with bushings designed for the purpose or did not have smooth, well-rounded surfaces on which the cords may bear. On February 11, 2014, at the Apra Phase 1 - US Naval Station Guam, P-204 Uniform-Tango Wharf improvement project; where the 3/4 black flexible cord entering the air conditioning system junction box for the the construction trailer did not have a bushing.
