29 CFR 1910.110(b)(14)(v): Filling of fuel containers for industrial trucks or motor vehicles from industrial bulk storage containers was not performed not less than 10 feet from the nearest important masonry-walled building or not less than 25 feet from the nearest important building or other construction and, in any event, not less than 25 feet from any building opening.                       On or about 2/18/2014 at the dock/waste compactor area; Employees filled portable propane containers used for powered industrial trucks on the dock area from a 250 lb. propane container that was approximately 5 feet from the dock wall of the building.

29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made. Aisle and passageways were not marked:    On or about 2/18/14 at the waste compactor dock area: Employees working in the waste compactor dock area are exposed to a struck-by hazard from the powered industrial trucks being operated in the area and the passageways not marked.

29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board.  On or about 2/9/2014 at the dock/waste compactor area: Employees standing on the platform operating the waste compactors without the opening of the machine being guarded to protect the employee from moving parts. One employee sustained an injury from the unguarded moving parts on the machine.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use:    On or about 2/18/2014 at the dock/waste compactor area: Oxygen, argon, and acetylene cylinders used for cutting and welding were stored together near oil containers without valve caps to protect the valves.

Located at 1884 Warrenton Highway in Thomson, GA on or about 2/18/14, the employer did not enter correctly the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2014.        a. On or about 2/18/14, four (4) cases for calendar year 2014 were not recorded properly, in that the number of days from work and other recordable cases blocks were marked.            1. Production Worker, 1/14/14: suffered a foot injury when a panel fell on his foot; was recorded as having 30 days away from work and other recordable cases.            2. Production Lead, 2/6/14: suffered a leg injury when struck by a forklift; was recorded as having 17 days away from work and other recordable cases.            3. Production Worker, 2/9/14: suffered a leg injury when caught-in the compactor; was recorded as having 9 days away from work and other recordable cases.            4. Material Handler, 2/10/14: suffered a back injury while pulling up a dock plate; was recorded as having 6 days away from work and other recordable cases.
