29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a.  On or about January 28, 2014, at the east side of the building, an employee setting up weather proofing material on a 6:12 pitch residential roof did not install his personal fall arrest system correctly and was exposed to a 10 foot fall hazard.    b.  On or about January 28, 2014, at the north side of the building, two employees setting up the drip edge on a 6:12 pitch residential roof did not install his personal fall arrest system correctly and were exposed to a 19 foot-8 inch fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  Employees performing roofing work on a roof 6 feet or more above ground are not protected from fall hazards by the use of a fall protection system.    The employer fails to assure effective protection against falls as required by the standard, as demonstrated on January 28, 2014, when employees were working on a residential roof at 13024 Pechora Ct., Jacksonville, Florida 32224 without fall protection and were exposed to a fall hazard of approximately 10 feet.  Professional Sunshine Roofing, Inc., previously committed a repeat violation of this standard on June 19, 2013, with respect to a workplace at 1398 Sunset View Lane, Jacksonville, FL, 32256, contained in OSHA inspection number 907966, citation 02, item 01.  The final order date was July 10, 2013.    Professional Sunshine Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 799521, citation 01, item 01, issued on January 8, 2013, with respect to a workplace at 13507 Montecito Place, Jacksonville, FL, 32224.  The final order date was January 28, 2013.

29 CFR 1926.1053(b)(15): Ladder(s) were not inspected by a competent person for visible defects on a periodic basis and/or after an occurrence that could have affected their safe use:  a. On or about January 28, 2014, at the east side of the building, a 30-foot aluminum extension ladder being used by employees to access the roof had three visible damaged rungs.
