29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator before the employee was fit tested or required to use the respirator in the workplace:   (a) On or about February 19, 2014 at 93A Route 84 at Greene County Emergency Medical Systems, Inc. and associated field locations, for employees required to carry and use N-95 respirators. The employer failed to provide a medical evaluation prior to requiring respirator use.    (b) On or about February 19, 2014  at 117 Mansion St., Coxsakie at Greene County Emergency Medical Systems, Inc. and associated field locations, for employees required to carry and use N-95 respirators. The employer failed to provide a medical evaluation prior to requiring respirator use.    (c) On or about February 19, 2014  at 151 Main St., Leeds at Greene County Emergency Medical Systems, Inc. and associated field locations, for employees required to carry and use N-95 respirators. The employer failed to provide a medical evaluation prior to requiring respirator use.    (d) On or about February 19, 2014  at 4786 Route 23, Windham at Greene County Emergency Medical Systems, Inc. and associated field locations, for employees required to carry and use N-95 respirators. 29 CFR 1910.1030(c)(1)(iv)(A): The review and update of the exposure control plan did not reflect changes in technology that elimnate or reduce exposure to bloodborne pathogens:  a)  On or about February 19, 2014 at 93A Route 84 at Greene County Emergency Medical Systems, Inc. and associated field locations, for employees exposed to bloodborne pathogen hazards when they respond to medical emergencies.  The employer failed to include use of safety needles as a control that reduces potential occupational exposure.
