The employer did not ensure that the release of hazardous energy could occur, while the employee performed maintenance on the Goliath Gantry Crane. This condition was observed on or about February 12, 2014. OR IN THE ALTERNATIVE: 29 CFR 1910.333(a)(1): Live parts to which an employee may be exposed were not de-energized before the employee worked on or was near to them: a) Eagle Bay (vessel), Goliath Gantry Crane (electrical Room), 2100 Kitty Hawk Ave, Philadelphia, PA - The employee was exposed to a shock/electrocution hazard as an energized buss bar (480 volts AC) was not de-energized. The employer did not ensure that the release of hazardous energy could occur, while the employee performed maintenance on the Goliath Gantry Crane. 29 CFR 1915.89(d)(1): The employer did not establish and implement written procedures to prevent energization or startup, or the release of hazardous energy, during the servicing of any machinery, equipment, or system. Each procedure shall include: a) Eagle Bay (vessel), Goliath Gantry Crane (electrical Room), 2100 Kitty Hawk Ave, Philadelphia, PA - The employer did not implement a written procedure to prevent the release of hazardous energy during the maintenance being performed on the Goliath gantry crane. This hazard exposed the employee to a shock/electrocution hazard as an energized buss bar (480 volts AC) was not de-energized. This condition was observed on or about February 12, 2014. OR IN THE ALTERNATIVE: 29 CFR 1910.333(b)(2)(i): The employer did not make a written copy of the procedures outlined in paragraph (b)(2) of 29 CFR 1910.333 available for inspection by employees and by the Assistant Secretary of Labor and his or her authorized representatives: a) Eagle Bay (vessel), Goliath Gantry Crane (electrical Room), 2100 Kitty Hawk Ave, Philadelphia, PA - The employer did not implement a written procedure to prevent the release of hazardous energy during the maintenance being performed on the Goliath gantry crane. This hazard exposed the employee to a shock/electrocution hazard as an energized buss bar (480 volts AC) was not de-energized. This condition was observed on or about February 12, 2014. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
