29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a) 10 - 16 Alfred Avenue, Somerset, NJ 08873: a) 10 - 16 Alfred Avenue, Somerset, NJ 08873: Employee was allowed to perform framing operations on the second floor eave approximately 24 feet from the ground below on a job-made work platform raised by the forks of a Pettibone Telescoping Lift Truck, without fall protection.    Condition occurred on February 12, 2014.

29 CFR 1926.602(c)(1)(viii): A lift truck, equipped with vertical only, or vertical and horizontal controls elevatable with the lifting carriage or forks for lifting personnel, did not have the following precautions to include (A) Use of a safety platform firmly secured to the lifting carriage and/or forks:  a) 10 - 16 Alfred Avenue, Somerset, NJ 08873: Employee was allowed to perform framing operations on the roof eave approximately 24 feet from the ground below on a job-made work platform raised by the forks of a Pettibone Telescoping Lift Truck, which was not secured to prevent cantilevering and was not secured to the forks.  Condition occurred on February 12, 2014.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:       a) 10 - 16 Alfred Avenue, Somerset, NJ 08873: Employees were observed performing framing operations at a height of approximately 24' - 30' to the ground below with no fall protection in place, exposing them to fall hazards.      Condition observed on or about February 12, 2014.
