Main Electrical Room:  On, or about, February 24, 2014, the following electrical enclosures were not provided with sufficient working space (depth):    1.      The Square D main service entrance enclosure was measured to be 29 inches from the break room wall.  The required distance was 42 inches.     2.      GE sub-panel that energized additional receptacles in the break room was measured to be 23 inches from the transformer.  The required distance was 36 inches.    B.     Building 808, Test Facility, Bay 4:  On, or about, February 27, 2014, a LCU test bank was located 30 inches from the face of the Cutler-Hammer Power Line Switchboard.  The required distance was 42 inches.

The salamanders were run using propane cylinders.   A permanent heating installation could have been installed, natural gas piping ran directly over the area where the salamanders were normally positioned during use.

The paint mixture components were Category 2 and Category 3 flammable liquids with flashpoints between 7 degrees, F and 99 degrees, F.    C.  Janitor's Closet:  On, or about, February 24, 2014, nonmetallic sheathed cable (e.g., romex) was run through the masonry block wall from a GE panelboard into an office area.  The cable was not protected against abrasions.
