29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: Warehouse: On or about November 15, 2025, a wall?mounted portable fire extinguisher was obstructed by a box of fire extinguishers, preventing immediate access. Employees working in the area were exposed to delays in firefighting and increased exposure to fire, heat, smoke, and toxic byproducts of combustion.
